Byron Family Medical Centre

At London Health Sciences Centre there are nearly 1-million annual ambulatory care visits, including clinics and day procedures. The ambulatory care portfolio consists of medicine and surgical services. Within the medicine services is family medicine, and two primary healthcare centres. The Byron Family Medical Centre is 1 of 2 sites which rosters 11,000 patients and sees approximately 22,000 patient visits annually. The Byron Family Medical Centre is an interdisciplinary healthcare team including Physicians, Nurses, Clerical, Physiotherapists, Dietitians, Social Workers, and Pharmacists and learners in all disciplines.

The Byron Family Medical centre is recruiting a temporary full time Nurse Practitioner to join this dynamic, interprofessional team. The Nurse Practitioner will work within a  collaborative practice environment, taking on a subset of rostered patients as MRP, supporting chronic disease management, health prevention and health promotion on topics such as diabetes education, chronic obstructive pulmonary disease, and congestive heart failure. In addition to clinical work, a portion of this role is dedicated to non clinical academic, teaching, and research practice for the medicine and family medicine program.

The Nurse Practitioner is a self-directed advanced practice nurse prepared at the graduate and post-graduate level, who demonstrates a high level of autonomy working to scope, both independently and within a collaborative practice model. Using advanced assessment skills and a sound knowledge of clinical therapeutics, the NP demonstrates a comprehensive knowledge base in primary care and exhibits a high degree of clinical competence in caring for patients across the lifespan. holistic care across the life spectrum. The NP has a focus on chronic disease management, episodic care, end-of-life comfort and palliative care, screening and health promotion counselling. The NP specializes in the management of diseases including (but not limited to) Diabetes, heart disease, and COPD.

The NP, Byron Family Medical Centre, will report to the Director of Ambulatory Medicine and Surgical Services, University Hospital, and will work collaboratively with members of the ambulatory medicine team. The NP will interact daily with the interprofessional team to manage, direct and provide comprehensive care for a this patient population. The NP works with key stakeholders to provide advanced care that optimizes patient outcomes and enhances safety, effectiveness and accessibility. The NP in this program will engage in scholarly activities and advance knowledge by providing clinical leadership, identify opportunities for improving patient care, advancing best practices, promoting professional education and engaging in research. The NP will provide education and collaboration to learners, including nursing students and medical residents in family medicine.

All duties and responsibilities of the NP conform to professional standards and hospital guidelines. Some travel will be required using a personal vehicle, as home visits and site visits for meeting and presentations are a part of this role.

Rate of Pay:                            $58.98 per hour - $71.85 per hour

Hours of Work:                        37.5 hours per week

Qualifications

 
  • Successful completion of a Master’s Degree in Nursing
  • Current registration in the extended class in any of the following specialties: primary care and in good standing with the College of Nurses of Ontario
  • Minimum of 5 years recent related experience with family medicine, gerontology, palliative care, and/or chronic disease management
  • Current member of the Nurse Practitioner's Association of Ontario (NPAO) or Registered Nurses Association of Ontario (RNAO)
  • Mandatory Current Professional Liability Protection
  • Current Basic Life Support for Healthcare Providers course: BLS-HCP (C)
  • Valid Drivers License (G class) with the ability to travel within the South-West LIHN
  • Demonstrated experience and leadership in clinical practice, education, program development and research
  • Expertise in functioning in an inter-disciplinary health care environment
  • Recent experience in family medicine, primary care, geriatrics, palliative care preferred
  • Ability to establish effective interpersonal relationships with colleagues and therapeutic relationships with patients and families
  • Expertise in critical thinking, clinical problem solving, negotiation and conflict resolution
  • Effective organizational, conceptual and leadership skills
  • Ability to provide project management?skills to promote?evidence-based strategies and initiatives for Family Medicine/ Primary care, and ability to work in partnership with relevant LHSC staff and physicians 
  • Experience in/or understanding research, program development and evaluation
  • Demonstrated excellence in interpersonal relations and communication skills, both oral and written
  • Demonstrated excellence as a role model for clinical leadership
  • Demonstrated experience with clinical teaching in an academic environment
  • Advanced presentation skills including the application of adult learning principles, change theory and systems theory in a clinical environment
  • Demonstrated excellence in clinical practice
  • Self-directed and function independently
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated skill in insight, self-awareness and self-management consistent with high emotional intelligence

  • Eligible for cross-appointment to an academic institution

  • Computer proficiency with Internet, Microsoft Office 365 including Outlook, Automated Clinical Systems and Virtual meeting platforms
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety

Immunization Requirements:

  • Provide Immunization or proof of immunity reflective of Ontario Hospital Association (OHA) Guidelines.
