Adult Mental Health

Area of Assignment– Consultation-Liaison

Consultation-Liaison is a specialized service within psychiatry that deals with the overlap of physical health and mental health care issues.  The primary mandate is to provide timely psychiatric consultation to those adult patients on medical / surgical units across the city. The work is predominantly inpatient-focused

The Nurse Case Manager will work with a diverse team in an inpatient setting providing case management to clients who suffer from various mental health diagnoses.  The incumbent will participate in various components of the program delivery including patient and staff education, family education, care planning, resource referral, liaising with the treatment team, facilitation of transfer to inpatient psychiatry unit etc.  This position also involves participation in detailed psychosocial and psychiatric assessment of clients using evidence-based practice. 

Regular Full Time

Rate of Pay:                         $39.57 per hour to $57.74 per hour

Qualifications

 
  • Current registration with the College of Nurses of Ontario
  • Current BLS for Healthcare Providers course: BLS-HCP(C)
  • Successful completion of a BScN degree
  • Canadian Psychiatric Mental Health Nursing Certification CPMHN(c) preferred
  • Minimum five (5) years recent, related experience working in an acute mental health care program preferably in an inpatient setting
  • Recent, related experience in crisis intervention skills
  • Knowledge of substance abuse preferred
  • Recent, related experience with delirium and related assessments
  • Expert knowledge of various acts including Mental Health, PIPEDA, PHIPA
  • Recent experience in case management and leading groups; willingness to learn and develop new case management skills
  • Ability to proficiently conduct a mental health assessment intervention and appropriately formulate disposition
  • Demonstrated knowledge of psychopharmacology and drug administration
  • Knowledge of community services and services across the mental health care continuum
  • Demonstrated knowledge of and experience with computer programs such as Microsoft Word, Excel, Outlook, Powerpoint, Database Software, Infomed, Cerner and Internet
  • Demonstrated strong communication (written & oral) skills including negotiation, consensus building and conflict resolution
  • Demonstrated skills in patient and/or staff teaching and assessment
  • Demonstrated effective organizational skills involving time management and prioritization
  • Demonstrated ability to manage change in a dynamic environment
  • Demonstrated commitment to professional development and self-directed learning
  • Demonstrated commitment to the employee performance development competencies
  • Demonstrated computer proficiency with Automated Clinical Systems
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety at LHSC
  • Demonstrated practice and commitment to LHSC’s vision, purpose and values
  • Demonstrated ability to attend work on a regular basis

Immunization Requirements:

Before beginning employment at LHSC, all new hires will be required to: 

  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis; 
  • Provide documentation of the Tuberculosis skin testing (two step);
