Multi-Organ Transplant Program

Reporting to the Manager, Multi-Organ Transplant Program, the Transplant Coordinator is responsible for coordinating the care of the kidney transplant recipients from the time of contact or referral throughout the transplant process (encompassing initial assessment, listing, transplantation, follow up care as required which includes accurate documentation/data collection).  The Transplant Coordinator provides education, is the key contact with patients/families and referring physicians, ensures that the transplant list is accurately maintained and is part of a 24-hour on call consultation service to transplant patients and healthcare professionals. This role will also involve supporting other Transplant Coordinators as needed. This role requires leadership and extensive interdisciplinary collaboration with numerous care providers to achieve this outcome.

Regular Full-Tiime

Rate of Pay:                         $44.33per hour - $55.41 per hour

Hours of Work:                     37.5 hours per week

Qualifications

 
  • Successful completion of a four-year Undergraduate Degree in Health Care related field
  • Current Certificate of registration from a college recognized under the Regulated Health Professionals Act
  • Current Basic Life Support for Healthcare Providers course:  BLS-HCP(C)
  • Minimum five (5) years acute care experience with a background in the care of transplant patients and disease management
  • Basic skill in the Microsoft Word, Excel and PowerPoint
  • Demonstrated ability to communicate effectively orally and in writing
  • Demonstrated facilitation and presentation skills
  • Demonstrated ability to work independently and as a team member
  • Demonstrated advanced accuracy and attention to detail
  • Demonstrated computer proficiency with Automated Clinical Systems
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety at LHSC
  • Demonstrated practice and commitment to LHSC’s vision, purpose and values
  • Demonstrated ability to attend work on a regular basis

Immunization Requirements

 

Before beginning employment at LHSC, all new hires will be required to:

  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis;  

  • Provide documentation of the Tuberculosis skin testing (two step);  

  • Provide proof of Health Canada-approved COVID-19 vaccination reflective of Ontario Health’s current definition of being fully vaccinated
