Salary Range: $39.16 - $50.77 /hour
Temporary employment anticipated to extend until November 19, 2027, subject to the availability of work.
A challenging and exciting opportunity exists to work in a collaborative interprofessional environment delivering services to rehabilitation patients in a specialized rehabilitation setting.
This is an ideal opportunity for a Social Worker that wants to be part of a team of rehabilitation practitioners who are considered leaders in the delivery of rehabilitative care and to actively participate in improving the outcomes of the patients we serve and to the health care system as a whole.
As a member of an interdisciplinary team, the Social Worker will be required to provide assessment, clinical intervention, consultation, counseling, discharge planning and education to individuals recovering from a neurological condition as well as their caregivers.
The current assignment is the Inpatient Stroke/Neurological Program.
Essential Qualifications
- Minimum 2 years of social work experience
- Valid registration with the Ontario College of Social Workers and Social Service Workers
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Knowledge of Mental Health Act, Health Care Consent Act, PHIPA and other relevant legislation
- Clinical experience in rehabilitation and practices related to neurological trauma
- Strong collaborative, communication, interpersonal, counseling and problem-solving skills
- Demonstrated ability to assess psychological needs of patients
- Demonstrated expertise in treatment planning
- Demonstrated strength in written/verbal communication
- Ability to execute early problem and needs identification through professional assessments and skillful implementation of all planned social work interventions in cooperation with the patient, family and interdisciplinary team
- Demonstrated skills in providing education to patients, families, the health care team and community partners
- Knowledge of community health service resources
- Ability to work independently and collaboratively with a wide range of health care professionals
- Excellent organizational, time management and communication
- Strong discharge planning skills
- Preferred Qualification
- Strong crisis intervention, advocacy, conflict management and facilitation skills
- Proficiency in French would be an asset
- Masters of Social Work
- Minimum five years of clinical experience

Immunization Requirements- Provide vaccination records or proof of immunity against measles, mumps, rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
- Provide documentation of the Tuberculosis skin testing
