Pharmacist - Inpatient Pharmacy Operations - Antimicrobial Stewardship/Infectious Diseases

At London Health Sciences Centre, the Pharmacist is responsible for the safe and effective use of medications for admitted patients. In meeting this responsibility, the Pharmacist reviews and monitors drug therapy; interacts with patients, physicians, nurses and other allied health professionals to resolve drug related problems; and counsels and educates patients regarding drug therapy.  Pharmacists also participate in the training and preceptorship of undergraduate and resident Pharmacist students; and may be an active participant in medication-based committee work.

As a core member of the Antimicrobial Stewardship team, the Pharmacist will optimize antimicrobial use through a variety of initiatives including, but not limited to, prospective audit and feedback rounds, developing clinical guidelines for prophylaxis and post-diagnosis treatment, retrospective targeted drug use evaluations, data analysis of compliance to existing initiatives, and policy work as a core member of the Antimicrobial Medication Committee. They will be highly engaged in staff education, including preceptorship and mentorship to pharmacy students and residents, providing education sessions to pharmacy and medical residents, and organizing and presenting at regional monthly stewardship rounds. They will endeavor to reduce the risk of antimicrobial-related complications such as multidrug-resistant organisms and C difficile infections and drug cost at an institution level.

As part of the Infectious Diseases team, the Pharmacist will optimize pharmaceutical care for patients consulted on by the Adult Infectious Diseases service. Responsibilities may include but are not limited to: rounding with the team and monitoring patients, providing clinical recommendations such as optimizing drug choice, dose, and route based on pharmacokinetic and pharmacodynamic principles, facilitating drug access during points of transition of care by leveraging resources such as the provincial EAP (Exceptional Access Program) and federal SAP (Special Access Program) programs, and providing patient and healthcare provider education on drug-related matters. They are expected to assist other pharmacists and committees as needed with antimicrobial-related items and will aim to reduce antimicrobial-related complications and cost at a patient level.

Clinical assignments may change based on operational needs, at the discretion of the pharmacy manager.

Temporary Full-Time

Hours of Work:          37.5 hours per week (guarantee of hours)

Duration:                   up to 18 months

Rate of Pay:              $52.16 per hour - $65.20 per hour

 

Qualifications

 
  • Baccalaureate Degree or Doctorate Degree (PharmD) in Pharmacy

  • Registered as an A Pharmacist with the Ontario College of Pharmacists (OCP)

  • Professional Liability Insurance as prescribed by the OCP
  • Hospital Pharmacy Residency certificate (Accredited Canadian Pharmacy Residency: ACPR) preferred
  • Up to one (1) year of recent, related experience

  • One (1) year of recent, related experience preferred
  • Experience in antimicrobial stewardship or infectious diseases preferred
  • Proficiency in OCP Standards of Practice for Pharmacists

  • Proficiency in Cerner PharmNet and Powerchart System

  • Proficiency in Kroll Pharmacy Computer System

  • Proficiency in the Microsoft Office suite of applications

  • Proficiency in Powerchart Oncology (Oncology Only)

  • Proficiency in Applicable Standards of Practice

  • Proficiency in NAPRA Professional Competencies

  • Detail-oriented with strong organizational skills

  • Ability to work in a high-stress, fast-paced environment while maintaining accuracy and professionalism.

  • Demonstrated practice and commitment to the principles of patient and family centered care
