Job Summary

 

This is an Ontario Health-funded role to support LHSC's Hospital to Home program.  The Admit/Discharge Facilitator facilitates and coordinates the movement of patients through the admission, transfer, and discharge processes with a focus on patients with complex discharge needs and participates in decision-making that results in an effective admission and discharge process and the optimal utilization of program and facility resources. As part of this program, this role will be tasked with working closely with our home care vendor to participate in regular rounding on their patients, support care escalation as needed, and be involved in transition planning at the end of the 16 week program.

Temporary Full Time 

Rate of Pay: $45.72 - $57.14 per hour

Successful candidates will be required to perform work within Ontario and must be available to work on location, as required.

Responsibilities:

  • In collaboration with members of the Health Care Team, facilitates appropriate matching of patient needs on admission to available beds and program resources
  • Liaises with internal and external programs and agencies on matters related to patient movement and facilitating /coordinating diagnostic tests and procedures as appropriate
  • Initiates and arranges repatriation requests and assists with transfers as necessary including transfers to and from other hospitals
  • Identifies and assists with solving system and process barriers to discharge
  • Collaborates with external agencies/LTC to discharge the patient to appropriate setting with support/resource in place to prevent readmission

Qualifications

 

 

  • Undergraduate Bachelor's Degree in a Health Care Discipline
  • Current registration in good standing with applicable professional college required
  • Minimum three (3) years recent related experience in a health care environment required
  • Five (5) years recent clinical experience working with inpatients preferred
  • Excellent written and verbal communication skills
  • Proficient knowledge of local and regional health care system
  • Proficient critical thinking and decision-making skills
  • Proficient people management skills
  • Ability to build collaborative partnerships
  • Proficient Microsoft Office skills
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety at LHSC

  • Demonstrated ability to attend work on a regular basis
